The IFPP Patient Check-In SMS program ("Program") is operated by Shokat Wellness Academy LLC ("we," "us," or "our") on behalf of Southern Interventional Pain Center. The Program sends automated SMS text messages to enrolled patients to support daily care plan check-ins as part of their treatment plan.
By signing the SMS consent addendum at your clinic visit and providing your mobile phone number, you expressly consent to receive automated text messages from Shokat Wellness Academy LLC at the mobile number you provided. Consent is not a condition of receiving medical treatment or purchasing any goods or services.
Frequency: Up to 2 messages per day (1 daily check-in prompt sent at approximately 10:00 AM ET, and 1 follow-up nudge if no check-in is recorded after 2 or more consecutive missed days).
Content: Messages will contain a summary of your most recent self-reported check-in data (pain level, medication adherence status, and care plan flags) and ask you to confirm or update. Messages do not contain diagnosis codes, medication names, or other protected health information from your medical record.
Message and data rates may apply. Standard messaging rates from your mobile carrier apply to all messages sent and received. Contact your carrier for details about your plan. We do not charge any additional fees for participation in the Program.

After opting out, you will receive a single confirmation message. No further messages will be sent unless you re-enroll. Opting out of SMS does not affect your enrollment in the IFPP mobile application or your medical treatment.

The Program is designed to work with all major U.S. mobile carriers. However, message delivery is subject to your carrier's network availability and policies. We are not responsible for delayed or undelivered messages caused by carrier issues.

No mobile information will be shared with or sold to third parties or affiliates for marketing or promotional purposes.
The IFPP Patient Check-In SMS program is a care plan support tool and is not a substitute for emergency medical care, professional medical advice, or direct communication with your healthcare provider. If you are experiencing a medical emergency, call 911 immediately.
The information you provide via SMS is self-reported and is used to support your care plan. Clinical decisions are always made by your healthcare provider based on a complete medical evaluation.
We reserve the right to modify these Terms and Conditions at any time. Updated terms will be posted at this page. Continued participation in the Program after changes are posted constitutes acceptance of the modified terms.
